Display Reservation Request Form

By submitting this reservation request, I accept full responsibility for the care of the display while it is in my possession.  I will follow all directions provided by Integrity for receiving, setup, breakdown, and shipping of the display.  When I receive the display, I will inspect it to make sure all contents are present.  After use, I will ensure that all contents have been repacked.  I will contact national Integrity immediately if any contents are damaged or missing.  I will ship the display back to national Integrity or forward it to the next borrower, as directed by Integrity, with a transit time of not more than 3 business days, via a commercial shipping company that provides package tracking and delivery verification.   I will insure the exhibit for $1500.  I understand that I or my chapter/network/circle will pay for all shipping charges.  I/we will also strive to make a contribution to national Integrity maintain the display.
